Student Requirements

Students — Welcome to Ascension Columbia St. Mary's! We are happy to have you as a guest at our facilities. Below you will find information that will be helpful to you prior to beginning your learning experience.

The information on this page is intended to provide the basic information, forms and web-based training needed to study, work and learn safely at Ascension Columbia St. Mary's (CSM). Prior to beginning your student experience at Ascension CSM, each student must complete each of the steps as outlined below.

 

Student Requirements

Step 1: Forms

Please complete the following forms and submit to your school coordinator or instructor.

 

Step 2: Mandatory Orientation Courses 
Please review the following courses. Note that your progress is not tracked in each course – if you leave a course in the middle, you will need to start again from the beginning. Be sure to leave adequate time to complete each course. Upon completion of the courses, be sure to complete the Attestation Form. CLICK HERE to complete the 7 part course and attestation.

 

Step 3: EHR And Other Clinical Training
All clinical students and others, as applicable, are required to complete the Electronic Health Record (EHR) and other web-based training modules found below.

ICD-10 Module (Mandatory for Residents, Medical Students and Advanced Practice Healthcare students in roles including, but not limited to, PA Students and NP Students)

Introduction to EHR

2014 Barcode Medication Administration & Positive Patient Identification: (Mandatory for Nursing and RT Students)

Safe Patient Handling SMART Moves Training Module (For anyone responsible for direct patient care or transport.) 

Severe Sepsis & Septic Shock: Priorities for Core Measure in Adult Sepsis Patients & Resuscitation Target 2016

Students will be granted access to the EHR once their information is entered into the student data base by the school coordinator and compliance forms are received. Usernames and Passwords will be sent to the Preceptor or Instructor.

Please Note: Confirmation of orientation needs to be completed once per calendar year (January-December). If you are starting a second clinical rotation since the beginning of the calendar year, you do not need to complete the confirmation. If you try to confirm completion for a second time, the database will recognize your name and state that confirmation has already been completed.
